Admissions Information
Pre-registration
Once you are scheduled for either an outpatient procedure or an inpatient admission, you will be contacted by a representative from our Admissions Department. The purpose of this call is to verify your demographic and insurance information Some procedures require insurance pre-authorization, which needs to be completed at least two days before your scheduled procedure; to assure you insurance pays the maximum allowed according to your policy benefits. If pre-registration is not done, our staff cannot verify your insurance or pre-authorize. If you are scheduled for surgery, you will receive an additional call from a nurse to discuss your procedure prior to your scheduled appointment.
Maternity Pre-registration
You can save time by pre-registering before your expected due date for delivery. You may pickup a pre-registration form from your physician’s office, or at Admissions in the Hospital main lobby, or fill out this Online Pre registration Form
Most insurance plans require pre-authorization for your admission for the delivery of your baby. Check with your employer’s benefits office or insurance company customer service representative to make sure that you have met their notification requirements.
If you are unable to complete the pre-registration forms in advance, you and your support person will need to allow extra time to complete the registration process when you arrive at the hospital. Please bring your insurance cards and photo ID for us to copy.

Pre-operative Appointment Information
Please arrive at least 30 minutes before your scheduled appointment. You may eat, drink and take your medications as usual on the day of your pre-op visit.
Parking on the day of your pre-operative appointment
Shuttle parking is available at the Main Entrance. Regular parking is also available in our lots around the hospital.
About your pre-operative appointment
After completing paperwork in Admissions/ OP Surgery Registration, you will be called to the Surgery department where a nurse will review surgery instructions and other information with you. A member of the anesthesia team will examine you and answer any questions you may have about the anesthesia you will receive during surgery. If ordered by your surgeon or anesthesiologist, you may have lab work, an EKG or x-rays performed during this visit.
This appointment will take a minimum of one hour so please bring something to occupy your time. The length of your appointment will depend on what tests your physician has ordered and what is required for your procedure. For your comfort, please bring a sweater or jacket, as it is sometimes cool in our waiting room.
Please bring the following to your pre-op appointment:
- insurance card, photo ID, or military ID if applicable
- any laboratory test results, x-rays or EKG’s
- a current list of your medications and dosages, including over-the-counter medications and vitamins
- Payment for any co-insurance or deductibles due as per your insurance carrier

Outpatient Surgery : On the day of your procedure/surgery
Enter the Medical Arts building entrance of the hospital. Take the elevator to the 2nd floor and report to the Outpatient Surgery Registration area.
What will I be asked to sign?
You may be required to sign the following:
- consent for treatment
- authorization to bill insurance
If you are a Medicare recipient, you will be required to sign, or answer:
- Medicare secondary questionnaire
- Medicare rights statement
Consent Forms
The registration representative will also ask you to sign a consent form, which is an agreement to allow our staff to treat you. A parent or guardian must sign for a minor. Additional forms may be required for special procedures during your stay. Your physician will discuss these with you.
Payment of Services
The hospital does insurance verification, so if you owe any co-insurance and/or deductibles you will be required to pay them prior to the surgery. If you need to make financial arrangements regarding your payment, you may contact a Financial Counselor prior to your surgical date at 870-508-1080.

It is your responsibility to make sure that the requirements of your plan have been met. If your plan's requirements are not followed, you may be financially responsible for all or part of the services rendered.
Your health coverage is a contract between you and your insurance company. While we will assist in expediting your claims, you are ultimately responsible for payment of your account.
Fees for the professional services of your attending physician, surgeon, radiologist, anesthesiologist or pathologist are not included in your hospital bill. Your will be billed separately by them for their services.
